According to EU regulation 2016/339 of 09 March 2017. Annex VI, 2.3.1. Checks on persons on private flights must follow next:

The Captain must transmit, prior to take off to DBV - LDDU, to e-mail adress: zlgp.dubrovnik@mup.hr of DBV – LDDU Immigration control, using form in attachment a general declaration in accordance with Annex 2 to the Convention on International Civial aviation and all data concerning passengers identities.
